Even when configuring shuffled playback for an Amazon Unlimited playlist in a preset, it always starts with the first track

That might suggest it has to start playback first then apply shuffle. Worth raising a ticket to WiiM and asking why. Also, I seem to recall it caches the playing queue not strictly a link to the playlist, so that might have an impact too.

This used to be a problem with Spotify and Alexa years back (and might still be there) and a workaround of adding a very short silent track at the start of a playlist masked it.

Well, in Alexa a shuffled playlist starts with a random track, and in the Amazon Music app too…
